• Immunizations and Records

New/transfer students are required to meet with the principal prior to admission.  A copy of their most recent report card should be provided to the school at that time. The school must receive student records from the previous school and have the opportunity to review those records before a student can be considered for enrollment.

All students new to Newman Catholic Middle & High School are required to have a complete physical, NOT a sports physical, within six months of the start of the new school year.  Please contact the Middle/High School Office at 715-845-8274 for more details and a physical form.

The Wisconsin Immunization Law requires students through grade 12 and children in day care centers to be immunized according to their age/grade level by the beginning of the school year.  Immunizations are required against measles, mumps, rubella, polio, diphtheria, tetanus and pertussis.
